Ofqual was told its algorithm was 'doomed' a MONTH ago but 'ignored the warnings' 

External advisers had told Ofqual that the formula for deciding the E-level and GCSE results of England's pupils was 'volatile' and ran the risk of producing erratic results, sources said.
