                Express Scripts Holding Co , one of the largest U.S. prescription benefits managers, will cover new migraine drugs from Eli Lilly  and Amgen Inc , but is excluding a rival medication made by Teva Pharmaceutical Industries Ltd  after price negotiations with all three manufacturers.
